A South Carolina woman is accused of using altered medical and accident paperwork to seek $8,140 in insurance benefits for a purported vehicle collision. The insurer paid $3,400 under hospital coverage but denied a separate $4,740 accident claim, according to sworn arrest warrants.

Trevon Rena Dawson, 49, of Greer, allegedly submitted the claims to ReliaStar Life Insurance Company, part of the Voya insurance group, on or about July 15, 2025. The applications concerned another person who was represented as having suffered injuries in a collision requiring hospitalization.

Investigators say the claims relied on two supporting records: a medical summary and an FR-10 form associated with the reported collision.

The South Carolina Law Enforcement Division charged Dawson on October 16, 2026, with presenting a false insurance claim and two counts of forgery. SLED announced the charges on October 20 after an investigation requested by the South Carolina Department of Insurance.

Claims Described a Hospital Stay at Greenville Memorial

The applications consisted of an Accident Claim (AC) form and a Hospital Confinement (HC) form. The warrants identify Greenville Memorial Vascular Services as the facility where the purported hospitalization occurred.

SLED’s warrant packet contains three separate affidavits addressing the insurance claim, altered medical paperwork and altered collision documentation.

Medical Summary Allegedly Contained Altered Physician Information and Hospital Dates

The felony forgery warrant, numbered 2026A4210500080, alleges that Dawson knowingly presented an altered medical summary as genuine to ReliaStar. According to SLED investigator Haneef J. Tull’s sworn affidavit, the document contained altered physician information and hospitalization dates. The affidavit states that Voya calculated potential benefits totaling approximately $8,140 using the submitted documentation and predetermined policy benefits.

Voya’s supplemental insurance materials explain that hospital indemnity insurance generally pays fixed benefits for qualifying stays, while accident insurance provides benefits for specified injuries and treatments. Both products are limited-benefit coverage, with payment determined by the relevant policy terms.

The FR-10 Allegedly Changed Details About the Collision

A separate warrant, numbered 2026A4210500081, concerns the FR-10 submitted with the accident claim. Investigators allege that Dawson knowingly presented an altered version containing changes to the identities of people involved and information about the collision.

Although the warrant refers to the document as an FR-10 collision report, the South Carolina Department of Public Safety identifies the FR-10 as a mandatory insurance verification form. The state’s detailed law enforcement collision report is the separate TR-310.

A Recorded Interview Became Evidence in the Fraud Investigation

The false-insurance-claim warrant, numbered 2026A4210500079, describes a voluntary interview recorded on audio and video. According to the affidavit, Dawson admitted that the claim was fictitious and that supporting documents had been altered. Tull identified Dawson’s admissions, electronic communications and electronic insurance records as sources supporting the investigation.

The three warrants were signed on October 14, 2026, by Magistrate Timothy Jerome Hall. Their return sections record delivery to Dawson on October 16, the date SLED identifies for the charges and her booking into the Spartanburg County Detention Center. SLED said the Department of Insurance would prosecute the case.

Two Felony Charges and a Separate Misdemeanor Forgery Count

South Carolina’s insurance-fraud statute, Section 38-55-170, classifies knowingly presenting a false claim involving more than $2,000 but less than $10,000 as a felony. Conviction can carry up to five years in prison and a fine determined by the court.

Under Section 16-13-10, forgery involving less than $10,000 is also a felony punishable by up to five years in prison and a discretionary fine. Forgery involving no dollar amount is a misdemeanor that carries a maximum prison term of three years and a discretionary fine.

That explains the separate treatment of the allegedly altered medical summary and FR-10. The charges remain accusations, and Dawson is presumed innocent unless proven guilty.

How to Check and Report Unfamiliar Insurance Claims

Policyholders who receive unexpected benefit notices can use their insurer’s official claims portal to check whether a claim has been submitted under their coverage. Voya’s Claims Center provides claim-status and document-submission resources for supplemental health coverage.

Anyone who finds a claim they did not authorize should contact the insurer through a verified customer service channel and preserve the relevant notices, claim details and correspondence. These records may help the insurer determine what was submitted and whether supporting documentation is authentic.

The South Carolina Department of Insurance accepts reports of suspected insurance fraud, including false claims and fabricated accident information. Its Insurance Fraud Hotline is 1-888-953-7283.
